Return To Play Form

The goal of this program is to collect de-identified data on concussions happening within USA Hockey. This information will allow us to better focus our educational efforts in the area of concussion management and return to play protocols to enhance the well being of all our players. The goal is to increase awareness of those protocols to drive compliance which ultimately benefits our players across the country.
